Buffs
A "buff" is any item/spell/potion/ability/etc that increases, decreases, or otherwise changes a skill, stat, or proficiency. Most buffs add to the total "buffed" score for an item, but there are exceptions. Potions, for example, actually raise base stats(so an Intelligence-enhancing potion raises your base(unbuffed) Intelligence temporarily.
Buffed
The term "buffed" applies to the total score after all buffs are applied.
Example: a character has a base STR of 10 and is wearing a Ring of Strength +2, their base STR is still 10, but they have a buffed STR of 12.
Modifiers
Modifiers are used in almost every aspect of calculations throughout the game. Modifiers are calculated for all skills, stats, and proficiencies.
Unless otherwise stated, a modifier is always the relevent score, divided by 5, rounded down to the nearest 0.5.
Example: A stamina of 15 yields an STA modifier of 1.5. A Stamina of 17 also yields 1.5, as does 19. Only a stamina of 20 raises the modifier to 2.
Buffed vs. Unbuffed
Some checks will work from a buffed or unbuffed scores. For example, the Exo-Mage uses unbuffed INT modifiers, making it very important for Exo-Mages to have higher base intelligence.
An Unbuffed Mod is the modifer for the unbuffed score. Some calculations might use the buffed modifier but the unbuffed score, or vice-versa(Again, such as Exo-Mages, who use the unbuffed INT Modifier but the Buffed Proficiency).
